url tags in postings here

Dang it... what's the url tag format for labeling a url in these posts?

Previously, when you clicked the little chain link icon in the edit page, you would enter the url and then another dialog would pop up to enter a label to present instead of the actual url.

It doesn't seem to do that anymore and I can't remember the tag format. I've tried about 4 variations so far, to no avail.
