Have you tried installing the w32codecs package? I believe that it has support for Real Media, if not maybe one of the gstreamer plugins? The ugly / bad ones
Failing that you could install RealPlayer for Linux http://www.real.com/linux I would have a look in your repo's before installing directly.

I have w32codecs installed. But still totem can't play .ram file.

I would consider it later should I fail making Kaffeine to work.
Installed Kaffeine and download essential-20071007.tar.bz2. However I found /usr/lib/win32 already existing and having files in it.
$ ls /usr/lib/win32/

Therefore I didn't decompress essential-20071007.tar.bz2 inside it.
Now sometimes I can play .ram file on Kaffeine and another time I can't. Similarly I can start Kaffeine and another time I can't. Restart PC is required. Would the problem be caused by without decompressing essential-20071007.tar.bz2 on /usr/lib/win32 ?
Kaffeine
settings --> xine engine --> parameters --> decoder and type /usr/lib/win32
already set.
Any advice? TIA

Hi deathadder,
If Kaffeine works .RAM file will be download on;
file:///tmp/kde-satimis/kaffeinewBP7bb.tmp
but repeatedly. I have to click stop after 1st download finished. Then Kaffeine starts to play.
If failure Kaffeine also starts with following warning;
Code:
Search for suitable codec?
The required software to play this file is not installed. You need to install suitable codecs to play media files. Do you want to search for a codec that supports the selected file?
The search will also include software which is not officially supported.
[Cancel] [Search]
Pressing [Search] then searching starts and finally ends with following warning displayed;
Code:
There is no matching application available.
[Cancel]
clicking [Cancel] kaffeine also starts to play without picture only sound.
On Gnome desktop
Applications -> Sound & Video -> Kaffeine
Kaffeine starts.
If failure to start there is no warning displayed.

Solved
Hi deathadder,
GStreamer needs following packages to play .RAM file;
gstreamer0.10-plugins-bad
gstreamer0.10-plugins-ugly
w32codecs
gstreamer0.10-plugins-ugly already installed as default. After installing the other 2 packages Totem now can play .RAM files
